Community Services

The Darke County Board of MR/DD Community Services Department assists individuals with services. Eight Service and Support Administrators (SSAs) assist individuals with services to ensure their health and welfare.

Residential Services

Supported Living - designed to maximize community inclusion by providing supports and assistance necessary to maintain a home in the community.

Individual Options (IO) and

Level 1 Waivers

Home and Community-based services funded through Medicaid, that allow an individual to reside independently with support and assistance in the community instead of residing in a nursing facility or a developmental center.

Service & Support Administration

Service and Support Administration (SSA) services are available to individuals eligible for county board services.

SSA services include function of referral, service placement, service coordination, crisis intervention, advocacy, incident tracking, as well as health and safety monitoring.

Family Support Services

Family support services may include respite care, adaptive equipment, home modification, counseling/training/education and special diets.
